  • Log in to the SV Parent Portal & select "Sports." 
  • Click the "Indicate Sport Tryouts" link and check off Ice Hockey under Winter Sports. 
  • Click the "Electronic Signatures Required for All Sports" link and complete Steps 1-6.
  • Ice Hockey is a Club Sport. See Paperwork Requirements for SVHA Players section or on the SVHA or SV Athletic website.
  • 6th Graders do not need to register on the portal as they don't have access.

Concussion Baseline Report Release Form (not needed for tryouts or to take the test - this is only if you need it released to your player's doctor) 

IMPORTANT DATES:   Physicals can be completed by SV or your private physician.   SV offers this service to student players if needed. Drug Testing & Concussion Testing MUST be completed by SV  Athletic Department with the exception of 6th Graders.

Dates for the 2024-25 season will be posted once SV Athletics releases them.

• PIAA Sections 1, 2, 5: (All Players) Completed by you and dated AFTER June 1, 2024. 
• PIAA Section 6  (ALL Players) Must be completed by a pediatrician/medical examiner and dated AFTER June 1, 2024.

  • 7th Graders and older: Student-athletes must take a Concussion Baseline Test every other academic year and it must be completed through the Seneca Valley School District.  Multiple test dates are offered prior to each sport season. If a student did NOT have a baseline test in 2023-2024, they must have a baseline test in 2024-25.  SV Athletic Office keeps these records.
  • 6th Grade ONLY:  PIHL requires all players to have a Concussion baseline on file.  Please get a baseline test through your health care provider or visit Heads Up Pittsburgh.  Send a receipt and/or paperwork showing completion of your baseline concussion test by Date TBD by SV Athletics to:
